You and Mark have hit a couple possibilities.

For everyday contacts, if there is are two birdies the right 
separation they can generate short-hand messages.  In a contest where 
more than one station may be calling it is conceivable that you might 
receive a short-hand message from the wrong station.

This is why its important in using WSJT to follow the message 
sequence completely.  You must see both calls before sending both calls + O's.
If you had both calls but the station next sends RO's or RRR's, I am 
suspicious of another station on the frequency.

As a QRP station this is common when I call a CQing station.  He may 
QRZ a couple times and then answers another station.  If he is a weak 
station some sequences may not decode (esp with QSB and Faraday), so 
I might miss his calling the other callsign.  Next I am hearing 
short-hand messages.  Well I never assume it was for me, but some 
stations might somehow make that error.

The lack of calls in the short hand messages can lead to mistakes, so 
one needs to be very sure that the station is sending to 
you.  Skipping 73's is another way a contact may be understood as 
complete when the other party has not completed.

I almost never send O's because I am usually the weaker station.  I 
often have a good copy long before the other station does.  When they 
send O's then I can send my RO's in confidence.

With DXpedition stations have we not some newcomers to eme?  If so 
they may be use to HF procedures that short-cut the call sign 
exchange.  On eme that will quickly lead to bad contacts.

Experience is the solution as in all things.  Whether CW, digital or 
whatever mode.

Not QRV due to elevation control problems (probably disconnected wire 
on actuator), so I will lower the array to fix it soon.
Hoping to be on this weekend to record some JT signals using my 
SDR.  I have a paper deadline looming.
